Under section 64 of the Freedom of Information Act 1982 (the Act), the Freedom of Information Commissioner is required to report annually to Parliament on the operation of the Act in all agencies. The FOI Annual Report is usually tabled during Parliament’s Spring Sitting.

To assist in the compilation of the 2012/2013 Annual Report, all agencies are required under section 64(2) of the Act to provide the Freedom of Information Commissioner with data regarding FOI matters handled in the period 1 July 2012 to 30 June 2013. To enable your agencies input to be included in the 2012/2013 Annual Report, your co-operation is sought in completing their return by no later than close of business 19 July 2013. This electronic survey has been designed to help you ensure that the data you provide in your return is valid.

On each of the following screens you will be asked to enter data required for your return into the fields provided. The survey validates the data entered as it is completed, avoiding the need to clarify inconsistencies after your return is submitted. This means that some of the screens will not allow you to continue until you have met certain requirements. For example, the total of personal and non-personal requests received must be equal to the total number of requests received at Step 7 before you can progress to Step 8.

At any point during the completion of your return you can save your progress by clicking on the [Save] button at the bottom of the screen. This allows you to exit the return, by clicking the [Exit] button, without losing the data you have entered therefore, your return can be completed over several sessions.

The ability to save partially completed returns means that you can start entering your list of exemptions at Step 11 or decision makers at Step 12 and save your data to allow you to return to complete your input at a later stage.

It is suggested that you save the link to this application to your internet favourites to allow you to return to it easily.

Individual screens can be printed by clicking [Print] button, although large amounts of text may not print via this method. The [Summary Report] button will provide a print preview of all information entered (including all text).

Clicking the [Next] button will validate the data in the current screen and advance to the next screen. Clicking the [Back] button will return to the previous screen and allow you to change the data previously entered. You should ensure that you [Save] your data before going [Back] as it will not be automatically saved.

Please note that this application will time out automatically after two hours if no data has been entered.
